Dr. Kevin D Stuart

Dear Patients:

I am writing to inform you that I will be closing my medical practice effective December 31, 2022. I will not be able to continue with your care or renew your prescriptions after that date. This has not been an easy decision for me, as caring for my patients has been a great honor and source of professional and personal satisfaction these many years.

Your medical records will remain in my EHR (Electronic Health Record). If you need to obtain a copy of your medical records or if you would like to have your records transferred to another physician, you must complete the attached form and return this form to the following address as soon as possible. 9460 No Name Uno, suite 130, Gilroy, CA, 408 847-1311

If you require continued medical care for a Gastroenterologic problem there are the following options:

  1. My colleague, Dr. Prince Shah, with whom I share an office, is accepting new patients
  2. Your Primary Care physician can help find and refer you to a new Gastroenterologist
  3. Gastroenterologists available in the area can be found at: https://gi.org/patients/find-a-gastroenterologist/

If you have had a colonoscopy or upper endoscopy with me and were told to have another exam, I request that you call the office when you receive this letter to confirm when you are due for your next colonoscopy/endoscopy.

I extend to you my best wishes for your future health and happiness. With warmest regards,

Kevin D Stuart, MD
